Pharmacy Residency Programs

Pharmacy Residency Program Information & Application PDF

Online Application

Pharmacy Practice (PGY1): Qualifications: Must be a self–directed individual with good interpersonal and communication skills, who holds a Pharm.D. degree, or equivalent experience.

Internal Medicine Specialty (PGY2): Qualifications: Must be a self–directed individual with good interpersonal and communication skills who holds a Pharm.D. degree, and has completed a Pharmacy Practice residency (PGY1) training.

Deadline
All application materials (application form, personal statement, curriculum vitae, letters of recommendation, transcripts) must be received by January 15th.

Procedure
  1. Return completed application form, personal statement and curriculum vitae to address listed below.


  2. Three letters of recommendation. Two letters should be from clinical clerkship preceptors. The third letter may be from a supervisor at your place of employment, another faculty member or preceptor, or another individual of your choosing. All letters of recommendation should be received by January 15th.


  3. For any pharmacy degree you hold, request that a copy of your transcripts be mailed to the address listed below.
