2008 Nomination Guidelines
WHO IS A HEALTHY WOMAN?
A healthy woman embodies the spirit of good health and has inspired others to do the same. She has made a commitment to being healthy herself and is educated about her health risks, gets appropriate screenings and makes educated lifestyle changes to improve her overall health.
AN ELIGIBLE NOMINEE MUST
- Currently live and/or work within the St. Louis metropolitan area.
- Be a woman 18 years of age or older as of April 1, 2008.
- Consent to the publication of her name and photo if selected as the recipient of the award.
- Be available to be interviewed by the Selection Committee if selected as a finalist.
- Attend the 2008 Conversations for Women event at St. Luke's Hospital on Saturday, April 26, 2008.
- Be willing to share their personal health story if selected as the winner of the St. Luke's Hospital Healthy Woman Award (HWA).
- Not currently be an employee of St. Luke's Hospital.

DEADLINE FOR NOMINATIONS
Nominations must be received by Monday, March 31, 2008 at 5 p.m. (CST)
HOW IS A NOMINEE SELECTED?
Each nomination form is reviewed by members of the HWA Committee. Once the finalists are selected, each is contacted for a personal interview with the Selection Committee. The group interview is approximately 20 minutes in length, with the winner notified by April 18, 2008.
Nominees will not be discriminated against due to race, color, or religious preference. St. Luke's Hospital employees are ineligible but are encouraged to nominate someone to receive the award.
HOW WILL THE NOMINEES/WINNER BE RECOGNIZED?
All finalists are encouraged to attend the Conversations For Women program on April 26, 2008. The 2008 St. Luke's Hospital Healthy Woman Award winner is announced at this event and her story is shared with the participants. The recipient of the St. Luke's Healthy Woman Award receives a personal gift from
St. Luke's Hospital and a certificate for a day of pampering at a local spa. Promoting women's health is the legacy the recipient provides to the community, as she plays a contributing role in upcoming women's health programs offered through St. Luke's Hospital.
