St. Luke's Heart Failure Center is a referral-based outpatient clinic designed to provide heart failure patients with effective, state-of-the-art disease management.
Heart failure disease management at St. Luke's Hospital offers a comprehensive, integrated program designed specifically for patients living with heart failure. Patients work with their physicians and the center's multidisciplinary team to develop an individualized plan of care that includes cardiac diagnostic testing, management of the patient's medication, laboratory monitoring, education and counseling and systematic telephone follow-up. Patients and physicians may access the heart failure disease manager, who is on call 24 hours a day. The disease manager assists in the treatment of the acute and chronic phases of this disease.
