In order to provide the most compassionate and appropriate care, St. Luke's has an inter-disciplinary team of individuals who believe deeply in the importance of the hospice and bereavement services we provide. Together, a care plan is developed in accordance with the patient's specific wishes and needs in mind.
The members of the St. Luke's Hospice Services team include:
Patient's choice of physician
Hospice registered nurse
Hospice medical social worker
Hospice spiritual care chaplain
Hospice grief counselor
Hospice aide
Hospice physician/medical director
Hospice trained and experienced volunteers
Physical, occupational or speech therapists as needed
