Goals of the Program
The Heart Failure Disease Management program goals:
- Evaluate and treat reversible causes of heart failure
- Maximize oral medication therapy
- Educate patients regarding causes of heart failure, the signs, symptoms and importance of early intervention, use of daily monitoring tools, medications, diet, exercise and activity, and treatment options
- Maximize quality of life for patients with chronic heart failure
- Minimize emergency room visits, hospital admissions and length of stay for heart failure
- Empower patients to be in control of wellness as related to heart failure
