Goals of the Program

The Heart Failure Disease Management program goals:

• Evaluate and treat reversible causes of heart failure
• Maximize oral medication therapy
• Educate patients regarding causes of heart failure, the signs, symptoms and importance of early intervention, use of daily monitoring tools, medications, diet, exercise and activity, and treatment options
• Maximize quality of life for patients with chronic heart failure
• Minimize emergency room visits, hospital admissions and length of stay for heart failure
• Empower patients to be in control of wellness as related to heart failure
