The Program's Individualized Therapeutic Options
The program's individualized therapeutic options include:
- Ongoing evaluation and management of patients with heart failure
- Individualized medical therapy
- 24-hour availability of nurse and physician treatment for Heart Failure Disease Management patients
- Provide aggressive outpatient treatment to: maximize oral medication therapy Administer infusion therapy as a bridge to transplant or maximal oral therapy
- Administration of intravenous medications for treatment of heart failure
- Assessment and modification of diet
- Education on heart failure, medications, diet, exercise and psychosocial adjustments
- Risk factor modification and health promotion
- Telephone monitoring and follow-up; collaborative appointments with heart failure physicians
- Collaboration and communication with primary care physicians
- Self-monitoring tools
- Provide for and direct a quarterly support group for patients and family members dealing with chronic heart failure
- Produce a quarterly newsletter that provides for ongoing information and education relating to heart failure
- Collect standardized and meaningful data to assure delivery of quality care
