Heart Failure Disease Management

Heart failure is a complicated, multi-system disease process for which many new therapeutic modalities are available. Early intervention and education allows physicians and patients to be partners in the successful management of this life-threatening disease.

St. Luke's Heart Failure Center is a referral-based outpatient clinic designed to provide heart failure patients with effective, state-of-the-art disease management.

Heart failure disease management at St. Luke's Hospital offers a comprehensive, integrated program designed specifically for patients living with heart failure. Patients work with their physicians and the center's multidisciplinary team to develop an individualized plan of care that includes cardiac diagnostic testing, management of the patient's medication, laboratory monitoring, education and counseling and systematic telephone follow-up. Patients and physicians may access the heart failure disease manager, who is on call 24 hours a day. The disease manager assists in the treatment of the acute and chronic phases of this disease.

Goals of the Heart Failure Center

  • Improve patient's functional status and quality of life.

  • Reduce acute care utilization through ongoing surveillance and symptom recognition. Reduce length of stay for acute admissions.

  • Assist in slowing disease progression by assuring proper therapeutic modalities.

  • Prolong periods of wellness and speed recovery during periods of illness.

  • Improve patient outcomes and reduce costs utilizing comprehensive and thorough assessment, treatment and follow-up.

  • Assure patient development of self-monitoring skills to promote an increased level of participation in their own healthcare.
