Diabetes Education
Cholesterol and Glucose Wellness Screenings
Get your cholesterol and glucose numbers. A one-on-one consultation with a registered nurse/health coach also includes blood pressure and body composition measurement. A 10-12 hour fast and advanced appointments are required.

Also available is the A1C test (a finger stick with rapid results). The test is designed for people at risk for type 2 diabetes or for people with a history of diabetes who wish to better manage their health.

Diabetes Education
St. Luke's Hospital Nutrition Wellness & Diabetes Center offers individual education sessions and group classes. A physician's order is required. Sessions may be covered under your insurance plan. Each person should contact their claim information hotline to determine coverage.

Diabetes Exercise Program
St. Luke's Wellness Center offers the Diabetes Exercise Program (DEP) for active or relatively healthy individuals with diabetes or pre-diabetes. This program is ideal for those who desire an independent home or gym-based program designed specifically for their needs. Those who have had a cardiac event in the past year or have a history of heart failure may be referred to outpatient cardiac rehab prior to joining the DEP.

Diabetes Group Classes
Our group classes will ensure your current diabetes management plan fits your lifestyle and health goals. Four classes cover a variety of topics:

Classes One: What is diabetes and monitoring blood sugars.
Class Two: Healthy eating and heart healthy tips.
Class Three: Medications and physical activity.
Class Four: Cooking demonstration.

Medicare and most other insurance companies will cover initial and annual follow-up diabetes education. Check with your insurance to verify coverage. A physician's order is required to bill y our insurance company. Call for more information and to register.

Diabetes Prevention Program Group Class
This is a new lifestyle change program that will deliver results. The program is led by the Centers for Disease Control and Prevention and is proven to help people with pre-diabetes delay or prevent type 2 diabetes. Please call for information and to register.

Healthy Weigh Program
Do you want to lose weight and improve your health? Have you been diagnosed with pre-diabetes and want to prevent type 2 diabetes? Do you want to lower cholesterol levels, control blood pressure and decrease joint pain? Join us for a 22-week program with proven results. Using the University of Pittsburgh Diabetes Prevention Program: Group Lifestyle Balance Program, our registered dietitian will help you implement lifestyle changes to reduce weight, improve your health and reduce your risk of getting type 2 diabetes. Healthy Weigh Program, One-on-One Option
Coaching and guidance for lifestyle changes and weight loss provided by our registered dietitian over the course of one year. BMI over 30 and physician order is required to enter the program.

Program is covered by Medicare with no deductible. If not covered by Medicare, check with your insurance for coverage.

How Sweet Are You? Know Your Risk for Diabetes.
To assess your risk, it's important to know your risk factors and your average blood glucose level. This screening is designed for people at risk of type 2 diabetes or for people with a history of diabetes who wish to better manage their health. The session includes a non-fasting A1C blood test (finger stick with immediate results), blood pressure and a one-on-one consultation with a certified diabetes educator.
