Cholesterol and Glucose Wellness Screenings

Get your cholesterol and glucose numbers. A one-on-one consultation with a registered nurse/health coach also includes blood pressure and body composition measurement. A 10-12 hour fast and advanced appointments are required.
Also available is the A1C test (a finger stick with rapid results). The test is designed for people at risk for type 2 diabetes or for people with a history of diabetes who wish to better manage their health.

Diabetes Exercise Program
St. Luke's Wellness Center offers the Diabetes Exercise Program (DEP) for active or relatively healthy individuals with diabetes or pre-diabetes. This program is ideal for those who desire an independent home or gym-based program designed specifically for their needs. Those who have had a cardiac event in the past year or have a history of heart failure may be referred to outpatient cardiac rehab prior to joining the DEP.

Healthy Weigh Program
Do you want to lose weight and improve your health? Have you been diagnosed with pre-diabetes and want to prevent type 2 diabetes? Do you want to lower cholesterol levels, control blood pressure and decrease joint pain? Our registered dietitian will help you implement lifestyle changes to reduce weight, improve your health and reduce your risk of getting type 2 diabetes. How Sweet Are You? Know Your Risk for Diabetes.
To assess your risk, it's important to know your risk factors and your average blood glucose level. This screening is designed for people at risk of type 2 diabetes or for people with a history of diabetes who wish to better manage their health. The session includes a non-fasting A1C blood test (finger stick with immediate results), blood pressure and a one-on-one consultation with a certified diabetes educator.

Intensive Behavioral Therapy (IBT) for Weight Loss
A personalized weight loss program for individuals with a BMI greater than 30 that focuses on behavior and lifestyle changes. Participants work one-on-one with a registered dietitian up to 22 visits in one year.
Program is covered by Medicare with no deductible. If not covered by Medicare, check with your insurance for coverage.
mystlukes Patient Portal Class
THIS CLASS HAS BEEN CANCELED
Learn how to better take control of your health through the use of our mystlukes patient portal. This class will teach you how to gain access to our patient portal, what the benefits are, what information you can find on the portal and how to access your information from a computer and even your smart phone.
You can come to class up to 30 minutes prior to be signed up for the portal, have help signing up for an email address and even how to download our app to your smart phone. Please bring your smart device and/or laptop if you have access. Please bring a photo ID.
