St. Luke's Hospital Nutrition Wellness & Diabetes Center offers in-person or virtual individual education sessions to support you on your wellness journey.
Fees: Insurance coverage for nutrition services vary. We will work with you to verify your insurance coverage. A physician's order is required to bill insurance. Self-pay options are available.

Diabetes Exercise Program
St. Luke's Wellness Center offers the Diabetes Exercise Program (DEP) for active or relatively healthy individuals with diabetes or pre-diabetes. This program is ideal for those who desire an independent home or gym-based program designed specifically for their needs. Those who have had a cardiac event in the past year or have a history of heart failure may be referred to outpatient cardiac rehab prior to joining the DEP. Diabetes Overview (Video)
Learn from St. Luke's endocrinologist, Karen J. Chang-Chen, MD, about diabetes treatment goals and management, strategies to reduce risk of complications and new technologies to monitor blood glucose.
Do you want to lose weight and improve your health? Have you been diagnosed with pre-diabetes and want to prevent type 2 diabetes? Do you want to lower cholesterol levels, control blood pressure and decrease joint pain? Our registered dietitians will help you implement lifestyle changes to reduce weight, improve your health and reduce your risk of getting type 2 diabetes. This program has proven results and can reduce the risk by up to 58% and can also improve outcomes for those already diagnosed.
Intensive Behavioral Therapy (IBT) for Weight Loss
A personalized weight loss program for individuals with a BMI greater than 30 that focuses on behavior and lifestyle changes. Participants work one-on-one with a registered dietitian up to 22 visits in one year.
Program is covered by Medicare with no deductible. If not covered by Medicare, we will work with you to verify your insurance coverage. A physician's order is required to bill insurance. Self-pay options are available.
St. Luke's Hospital Nutrition Wellness and Diabetes Center Medicare Diabetes Prevention Program (DPP) is a Medicare-covered lifestyle change program for participants 60 and older to help lower the risk of type 2 diabetes. This program is backed by years of research and recognized by the Centers for Disease Control and Prevention and is proven to lower the risk for type 2 diabetes among participants who follow the program and lose 5 - 7 percent of their body weight.
Participants will meet in a group setting for 16 core sessions over a six-month period. The sessions cover a number of topics including making lasting changes around diet and exercise, and strategies to control your weight. Once you complete the core sessions, you'll receive six monthly follow-up sessions to help you maintain your healthy habits.
Please see the attached flyer for more information on the program and eligibility. This program is covered by Medicare Part B for people with prediabetes who are overweight.
Learn the benefits of using the mystlukes portal. The mystlukes portal is a secure online tool with convenient 24/7 access to your personal health records that is free and easy to use. Patients are able to communicate directly with their physicians, request appointments, view visit summaries and pay bills.

St Luke?s Hospital invites you and your family to Wellness Head to Toe for Diabetes Control. Individuals and families come in all shapes and sizes, and so do their healthcare needs. This FREE in-person program will empower you with knowledge and resources to help you keep your health on TRACK to prevent and take control
The program will start with a health fair featuring vendors, health experts, screenings and healthy snacks, followed by a speaker panel of St. Luke?s specialists to answer your questions. Plus, you could win attendance prizes!
