Hospice Services Team

In order to provide the most compassionate and appropriate care, St. Luke's has an inter-disciplinary team of individuals who believe deeply in the importance of the hospice and bereavement services we provide. Together, a care plan is developed in accordance with the patient's specific wishes and needs in mind.

The members of the St. Luke's Hospice Services team include:

• Patient's choice of physician
• Hospice registered nurse
• Hospice medical social worker
• Hospice spiritual care chaplain
• Hospice grief counselor
• Hospice aide
• Hospice physician/medical director
• Hospice trained and experienced volunteers
• Physical, occupational or speech therapists as needed
• Registered dietician as needed

Reza Jalal, MD, is the medical director for Hospice Services. He earned his medical degree from Kilpauk Medical College, University of Madras, India, and completed his Internal Medicine residency at the University of Connecticut Health Center. Dr. Jalal is board certified in Internal Medicine and Hospice and Palliative Medicine.
