
Outpatient Care Managers
At St. Luke's, our goal is to provide the best care possible and to help you stay healthy. For certain patients with high-risk discharges from St. Luke's Hospital, we connect you with an Outpatient Care Manager to provide continued support as you transition to home or to another care setting. Our team of Outpatient Care Managers are among the best in Missouri in helping patients avoid readmissions.
How will you benefit from an Outpatient Care Manager?
Our Outpatient Care Mangers will assist with the following:
- Provide education to you, your family, your guardian and/or your caretaker to support self-management, independent living and activities of daily living
- Establish or re-establish referrals and arrange for needed care and community resources
- Coordinate communication between providers, agencies and community services that you use
- Empower you to take an active role in self-management and work towards mutual goals
What happens next?
Some patients 65 and older hospitalized at St. Luke's with a diagnosis of heart failure, pneumonia, myocardial infarction or chronic obstructive pulmonary disease may qualify for the 30-day program. The patient will receive a phone call from one of the Outpatient Care Managers within 48 hours of discharge to review the recent hospitalization and enroll in the program.
What is an example of the value of the Outpatient Care Managers?
After a procedure or hospital stay, patients are often discharged with prescriptions to fill at their local pharmacy. Sometimes a patient may have difficulty filling important prescriptions that prevent dangerous complications. A care manager can help the patient identify ways to access the medication timely. In the past, our care managers have helped patients enroll in medication financial assistance programs or obtain samples from their doctor until the pharmacy could fill their prescription.
