Heidi Jacobs, MSN, FNP-C, is a Family Medicine nurse practitioner at St. Luke’s. She provides comprehensive primary care for patients of all ages, managing acute concerns, chronic conditions, preventive needs, and minor procedures with a focus on timely, evidence-based treatment. Drawing on a diverse background that spans oncology, telemetry, critical care, and behavioral health, she approaches patient care with strong clinical versatility and a deep understanding of complex recovery needs.
Heidi earned her Master of Science in Nursing as a Family Nurse Practitioner from the University of Central Missouri and completed her Bachelor of Science in Nursing at Saint Louis University.
She is committed to providing compassionate, patient-centered care and strives to create a welcoming, supportive environment. Heidi values clear communication, shared decision-making, and empowering individuals to take an active role in their health.
